Does following a Mediterranean diet reduce the risk of dying from any cause?

Plain-Language Answer Yes, with strong and consistent support. People who more closely follow a Mediterranean-style diet, rich in vegetables, fruit, whole grains, legumes, olive oil, and fish, with limited red and processed meat, have a lower risk of dying from any cause over long-term follow-up compared with people who follow it less closely.

Why This Matters

All-cause mortality is the broadest, hardest-to-manipulate outcome researchers can study. A dietary pattern associated with lower all-cause mortality across many independent cohorts, in different countries, is a stronger signal than a single trial on one biomarker.

What the Evidence Says

A widely cited 2008 meta-analysis pooled 12 prospective cohort studies covering more than 1.5 million adults. Among 8 cohorts specifically evaluating overall mortality (514,816 subjects, 33,576 deaths), a two-point increase in Mediterranean diet adherence score was associated with a 9 percent relative reduction in the risk of death (pooled relative risk 0.91). A larger 2018 umbrella review of 13 meta-analyses of observational studies and 16 meta-analyses of randomized trials, covering more than 12.8 million subjects, found consistent, robust associations between higher Mediterranean diet adherence and lower risk of overall mortality, cardiovascular disease, and several other chronic disease outcomes.

This is one of the most consistently replicated findings in nutritional epidemiology, supported by both large observational meta-analyses and, for cardiovascular outcomes specifically, randomized trial evidence (including the PREDIMED trial). The main caveat is that most of the mortality evidence is observational, so it demonstrates a strong, consistent association rather than proof that diet alone causes the reduction in every individual.

What Is Still Uncertain

  • Most of the underlying data is observational, so it shows association, not guaranteed individual causation.
  • Adherence to the diet is usually self-reported via food-frequency questionnaires, which introduces measurement error.
  • People who already eat this way may differ from those who do not in other health-relevant ways (income, smoking, activity level), even after statistical adjustment.
  • The traditional Mediterranean pattern studied includes lifestyle factors beyond food choice, such as shared meals and physical activity, that are harder to isolate.

Prevention and Risk-Reduction Context

Mediterranean-style eating is one of the most evidence-backed dietary patterns for reducing cardiovascular and all-cause mortality risk. It is not a guarantee against disease, but current evidence supports it as a reasonable, low-risk pattern to adopt for long-term risk reduction.
